Birthdate must be 10/1/21 or later.

Enrollment Forms
Please use the links below to print off many of the necessary forms needed when enrolling at Creative Kids. Please note that additional forms may be required, determined by age or needs of your child. The are in pdf format. If you are unable to open these, we can provide you with hard copies at the center.
